Do I have to complete forms before my appointment? +
It’s not strictly required, but strongly recommended. Completing the new patient registration form in advance dramatically reduces check-in time and ensures Dr. Gabal has your medical history ready when she sees you — meaning more of your appointment is spent on care, not paperwork.
How do I open and complete the PDF form? +
Most modern web browsers, smartphones, and computers open PDFs automatically. You can either fill it in electronically (the PDF includes fillable fields) and print it, or print a blank copy and complete it by hand. If you need a PDF reader, download Adobe Reader for free.
How do I submit my completed forms? +
You have three options: bring them with you to your appointment, upload them through the patient portal, or call our office at (949) 825-7650 for fax instructions. All three are secure and routed directly into your chart.
Is the patient portal secure? +
Yes. Our patient portal is HIPAA-compliant and uses encryption to protect your personal health information. Forms uploaded through the portal are routed directly to your chart and viewable only by your care team.
What if I make a mistake on the form? +
Don’t worry — minor corrections are fine. You can cross out errors and write the correction next to them, or download a fresh blank copy and start over. If you have questions about anything on the form, our staff will happily help you complete it at your appointment.
Can someone else complete the form for me? +
Yes — a family member, caregiver, or legal guardian can help complete your form, but the patient (or their legal representative) must sign and date the form themselves. If you have specific accessibility needs, please call us at (949) 825-7650 and we’ll make accommodations.
