Soothing the bladder from within
Intravesical heparin cocktail is a targeted therapeutic instillation delivered directly into the bladder to relieve chronic symptoms like urgency, frequency, and pelvic pain — most commonly used in the treatment of interstitial cystitis (IC) and painful bladder syndrome. The procedure is quick, in-office, and you can drive yourself home.
Relief that’s delivered where you need it
Living with chronic bladder symptoms — constant urgency, frequent bathroom trips, pelvic discomfort — is exhausting. Oral medications can help, but they have to travel through your whole body before reaching the bladder. Intravesical heparin cocktail takes a different approach: the medication is placed directly inside the bladder through a small catheter, putting it right where it’s needed.
This treatment is most often used for interstitial cystitis (IC) and painful bladder syndrome, and is part of a broader plan that may include dietary changes, oral medications, and other supportive care.
Three chronic bladder symptoms
Heparin cocktail instillations are typically used to address the most disruptive day-to-day symptoms of chronic bladder conditions.
Urinary urgency
That sudden, intense need to go right now. The kind that interrupts meetings, conversations, and sleep — and forces you to constantly know where the nearest bathroom is.
Urinary frequency
Needing to urinate far more often than normal. Multiple bathroom trips per hour, waking up repeatedly at night, planning your day around restroom access.
Pelvic pain
Persistent discomfort or pain in the pelvic region. Can range from a dull ache to sharp, burning sensations that affect comfort during everyday activities.
A carefully formulated therapeutic blend
The “cocktail” typically combines several medications, each playing a specific role to soothe the bladder lining and provide both immediate and lasting symptom relief. Your provider tailors the specific formulation to your situation.
Heparin
A medication that helps protect and restore the bladder’s protective lining, which can become compromised in chronic bladder conditions like IC.
Anesthetic
A local anesthetic typically included to provide rapid pain relief and reduce burning during and after the instillation.
Buffering agent
An alkalinizing buffer to optimize the absorption of the anesthetic and reduce bladder irritation during the treatment.
A targeted, in-office instillation
A small flexible catheter is gently inserted into the bladder through the urethra using local anesthetic to minimize discomfort. The heparin cocktail solution is then slowly instilled, and the catheter is removed.
You’ll be asked to hold the medication in your bladder for a period of time — typically 30 to 60 minutes — so it can soothe the bladder lining. You don’t need to remain in the office during this time. Your provider will give you specific instructions on the hold time and bathroom timing.

Usually a series, not a single visit
Heparin cocktail instillations are most often performed as a series of treatments — typically weekly for a defined number of weeks — to build up cumulative benefit. Some patients may continue with maintenance treatments at a less frequent interval once initial improvement is established.
Your provider will create a treatment schedule based on your specific situation, your symptoms, and how you respond to the first few sessions. Improvement is typically gradual and builds over the course — rather than from a single visit.
